The Mechanism Behind rTMS for Migraine

Leahu et al. (2021), published in Brain Stimulation (https://doi.org/10.1016/j.brs.2021.10.383) conducted a double-blind randomised control study involving 65 patients with episodic migraines. The study used a Magventure machine and compared real rTMS treatments using a circular coil to sham treatments. The protocol involved six sessions over two weeks, with the coil moved across the surface of the head in sweeps and spot burst stimulations. The real treatment group showed a significant reduction in migraine days from 7.6 to 4.3 days per month, with a 42% reduction in migraine attack frequency.
Importantly, no serious adverse events were reported, and minor side effects like headache and dizziness were comparable between the treatment and sham groups.
Another study by Milnik et al. (2013) in Germany (https://doi.org/10.1016/j.neulab.2012.10.002) used a similar high-frequency stimulation protocol. This non-blinded trial demonstrated that rTMS could be both a preventive and acute treatment for migraines, significantly extending the migraine-free interval and alleviating symptoms.
What to Expect During Treatment
Patients typically undergo rTMS treatment twice a week over three weeks, totalling six sessions. The frequency of follow-up treatments depends on individual progress. Magventure advises that for most people, returning in just under a year for another batch of six treatments is appropriate. Some patients may require "top-up" treatments. Patients can continue with their usual medications as there is no interaction with other types of treatment.
